Athletic Clearance Information

St. Bernard High School is excited to offer the following sports for the 2021-22 school year:

 

Football Girls and Boys Volleyball Girls and Boys Soccer Girls and Boys Cross Country Girls and Boys Basketball

Girls and Boys Track and Field Baseball Softball Sideline and Competitive Cheer

 

 

If your student is planning to participate in any of our athletic programs during the 2021-22 school year, a complete set of medical, permission and emergency information forms must be on file with us. This includes a current physical and an image of your student-athlete’s medical insurance card.

 

 

This paperwork must be completed and approved or your student-athlete will not be considered “cleared” for participation (practices or games). Everything is done electronically:

• Please go to the website: www.AthleticClearance.com and create an account
• From there you can fill in, upload and digitally sign all the necessary paperwork, which will then automatically be submitted to the Athletic Department
• On this page are links to the following documents:
• a step-by-step explanation on how to start your Athletic Clearance account
• blank physical forms that you can print and give to your physician to fill out
• Once received, we will review and clear your student-athlete
• If anything is incomplete or missing, you will receive an email with further instruction
